  • Current Curriculum Vitae
  • Minimum of two recent satisfactory clinical references (including telephone and fax numbers)
  • GMC number (if and when registered)
  • Laboratory evidence of Hepatitis B immunity, Hepatitis C (with English translation, where appropriate). Results should not be older than six months and Hepatitis B results must state both the sAg and anti-Hep B titre and not just proof of vaccination
  • Professional indemnity cover status, if held (e.g. MPS or MDU)
  • CPR qualifications (ALS/EPLS) or equivalent
