At least two original and recent references from satisfactory referees. One should cover the last six months and at least one should cover work done during the previous two years.
Laboratory reports for Hepatitis B and Hepatitis C, with English translations where appropriate. Results should not be older than 6 months and Hepatitis B results must state both the sAg and anti-HepB titre and not just proof of vaccination. (English translations please, where appropriate).
GMC Status – please give number if you already hold registration. If you do not already have Full GMC registration, you will need to view the GMC website at www.gmc-uk.org
Resuscitation qualifications (ALS/EPLS ) and the dates that current certificates expire. All certificates should have a year to run before their expiry date ; if they haven't new/refresher courses should be booked within the 12 months following that expiry date
Chest X-ray report (no more than three months old)
Police Clearance Report / Certificate of Good Standing (If you have worked in the UK within the last 12 months, you will also need to provide a CRB certificate). Certificates of Good Standing (Police Clearance Certificates) will be required from your home country if you have not worked in the UK before. We will help you to obtain these documents if you require.
